Developed by the European Health Management Association, an organization supported by the European Commission's "Quality of Life and Management of Human Resources Program", this resource addresses the issue of integrated care for older people with long-term, complex and multiple needs.
Recognizing the complexity of implementing integrated care, the resource was designed for managers for care providing organizations and those involved in policy development for older adult services. It aims to provide the main concepts, questions and considerations manager require s for strategic implementation, and includes definitions, organizational structures, needs assessment, care pathways and case management tools. It also provides further readings and web resources. While the document is aimed primarily to a European audience, it will likely be of interest to those providing services to the elderly in North America.
